Desired Learner Outcome(s):

• All sites will have at least 2 of the previously listed service lines fully implemented with STAAR pathways by the end of 2021.
• We will improve pre-op malnutrition screening by 25% by the end of 2021.
• We will reduce AKI by 2% by 2021

Target Audience:
Physicians: primary care, internal medicine, family medicine, surgeons, all specialties referring to surgery, anesthesiology.  Nurses: pre-surgical testing, pre-op, surgical, post-op.  Other providers: physical therapy, occupational therapy, dietitians and other interested professionals.
