Overall Objectives:

  •  Define collaborative care and identify situations that would be appropriate for referral to behavioral health for collaborative care.
  •  List ways in which behavioral health intervention can aid in providing improved quality of care and outcomes for disease management.
  • Recommend ways a physician can address behavioral health in a routine visit, follow up or urgent care visit.
  • Distinguish risk factors for poor disease outcomes that are related to treatable behavioral health issues.
