Overall Objectives:

At the end of this session, learners should be able to:

The objective of these sessions is to bring together a multi-disciplinary group of Clinicians and subject matter experts to review and collaborate on care plans for high and very-high risk patients to reduce unnecessary ED visits and admissions and in turn reduce cost of care for this vulnerable population. Specific tactics may include (but are not limited to) enrollment with Care Management, optimizing visit cadence with both PCP and Specialty, utilizing additional wrap around services such as CDM and dietary, and bringing attention to/addressing care gaps or chronic conditions.
