AAH Nursing Grand Rounds Recordings: Researching Reasons for Heart Failure Readmission

Heart failure (HF) is a staggering clinical and public health problem with many efforts to limit rehospitalization. These presenters will share findings from a mixed methods study using data from HF patient, caregiver, physician, and nurse interviews and chart reviews to identify and compare reason for HF readmission and perceptions about preventability.
