  1. Describe what a Hierarchical Condition Category (HCC) is
  2. Understand Documentation Requirements 
  3. Identify diagnoses that increase the severity of the patients illness/RAF Score
  4. Knowledge of Epic tools available for the Hierarchical Condition Category (HCC) work
