Functional Assessment and Transitions Planning for the Hospital Patient

Geriatrics for Advanced Practice is a unique educational activity that is intended to enhance the knowledge and ability of advanced practice providers to be able to contribute to the health of a population and provide optimal evidence-based care for the high-risk population of older adults. 

Target Audience

This course is intended for Physicians (MDs & DOs), Advanced Practice Clinicians (NPs, PAs, CNS), PharmD, and RNs.

Learning Objectives

At the end of this session, learners should be able to:

  1. Understand functional needs during the acute care transitions planning.
  2. Integrate what ‘matters most’ as they develop safe transitions planning.
  3. Integrate medication management strategies during safe transitions planning.
