A Paralyzing Problem: Dosing Considerations for Rapid Sequence Intubation in Obesity

A discussion of common dosing discrepancies and subsequent treatment failure in obese patients with rapid sequence intubation (RSI) medication management. This discussion will include a background review of current guideline-directed therapy, as well as an in-depth assessment of pharmacokinetic and pharmacodynamic characteristics related to commonly utilized RSI agents. Additionally,  guidance will be offered on how to optimize patient outcomes based on medication-specific characteristics and physiologic alterations in obesity.

Learning Objectives

At the completion of this activity, the participant will be able to

  • Assess indications for rapid sequence intubation (RSI) and review current guideline-directed medication management
  • Acknowledge physiologic limitations and barriers to treatment in obese patients undergoing RSI
  • Evaluate pharmacokinetic/pharmacodynamic variations in RSI medications altering efficacy and safety in obese patients
  • Clarify dosing recommendations for RSI medications in obesity based on PK/PD variations
  • Identify special considerations for post-intubation care and RSI agent-reversal in obese patients
