Perspectives on Discharge Planning

This seminar is designed to present clinicians with novel best practices in discharge planning. We will hear perspectives on scripting, preparing the patient for discharge, measuring progress and goal setting from five clinicians from various settings. This seminar will end with a panel Q&A.

Target Audience

This course is intended for physicians, psychiatrists, psychologists, social workers, nurses, and other allied health professionals who deal with mental health issues and addiction medicine.

Learning Objectives

At the end of this session, learners should be able to:

  1. Apply 3 discharge planning best practices to patient interactions
  2. Demonstrate effective scripting surrounding discharge
  3. Establish framework for extenuating circumstances while considering discharge readiness of patients
