This course will provide tools for how to screen patients for social determinants/drivers of health factors.

 

Target Audience

This course is intended for nurses, community health workers, and social workers.

Learning Objectives

At the end of this session, learners should be able to:

1. Identify the basics of social drivers of health and why we screen as an organization.
2. Identify effective communication skills to screen patients for social drivers of health factors.
