Too Much of a Good Thing - Deprescribing Unnecessary Medications During the Inpatient Admission

An inpatient admission provides numerous opportunities for removing harmful or unnecessary medications for patients.  Many medications that were started inpatient and chronic outpatient medications are no longer needed by patients.  During an inpatient admission, the pharmacist plays a key role in identifying these medications and taking action to get these medications deprescribed.​​

At the completion of this activity, the participant will be able to

  • Recognize the barriers to deprescribing in the hospital setting
  • Identify unnecessary or harmful medications that warrant deprescribing
  • Determine deprescribing opportunities during transitions of care
  • Evaluate tools used throughout transitions of care for deprescribing​​
