Aurora Protocol Updates and Revisions - April 2020

Attached are revised protocols and new protocols mainly addressing COVID 19 needs but some addressing recent changes in State Scope of Practice.  There is a signatory page that lists the protocols and a brief synopsis of major changes (pasted below).  The service director also needs to sign the signatory page and keep a copy with each member's training records.

 

Allergy & Anaphylaxis 2020 replaces previous Allergy & Anaphylaxis protocol

Adds MDI as alternative to Albuterol nebulizer.  At Intermediate and Paramedic level, modifies dose of IV epinephrine.

Asthma/COPD 2020 replaces previous Asthma/COPD and Pediatric Asthma protocols

Adds MDI and IM epinephrine as alternative to Albuterol nebulizer.  At Paramedic level, adds magnesium.  At Intermediate and Paramedic level, allows IV epinephrine.

COVID-19 Non-Transport Assessment April 2020 protocol and screening form replaces COVID-19 Non-Transport and screening form (dated March 21)

Softens language about non transport and if patient still wants transport despite meeting low risk criteria, to call Medical Control for decision about transport.

EMS MDI training and MDI Spacer- new

Fever 2020 protocol- new

Allows providers to give acetaminophen, ibuprofen or ketorolac (Toradol) to patients with fever who are uncomfortable from the fever.

Sepsis Septic Shock 2020 replaces previous Sepsis Septic Shock protocol

In addition to fever protocol changes, increases fluid bolus to 30 ml/kg and adds bolus IV epinephrine instead of dopamine.

Shock 2020 replaces previous Shock and Pediatric Shock protocols

Adds bolus IV epinephrine instead of dopamine.

Wisconsin Scope of Practice 2020 replaces previous Scope of Practice for EMR through Critical Care Paramedic.

Adds acetaminophen, ibuprofen and ketorolac to AEMT and above scope of practice.  Removes Dopamine from Paramedic scope of practice.  Allows EMS personnel to assist patients with their own medications with Medical Control approval.

Requires AEMT and above services to carry acetaminophen and at least one NSAID (ibuprofen, ketorolac).  Requires magnesium to be carried by Paramedic services. Removes Dopamine from list of Paramedic medications.  Hydromorphone (Dilaudid) not allowed at the EMT-Intermediate level.

Once signed by Medical Director and Service Director, these protocols are in effect.  There is an expected lag in between protocol in effect and everyone trained, familiar and implementing these protocols, during which following old or these new protocols is acceptable.  Each service can decide on date of complete transition with all services transitioned by July 1st, 2020.

Target Audience

This course is intended for those licensed at the EMR level through Paramedic. This course satisfies the requirements for the State of Wisconsin EMT Flexible Refresher Renewal.
