GAP 3.0 Interactive Virtual Session 2020

Geriatrics for Advanced Practice is unique and interdisciplinary educational activity that is intended to enhance the knowledge and ability of advanced practice providers to be able to contribute to the health of a population and provide optimal evidence-based care for this high-risk population. ​

This session will be 2 hours in length, you are required to attend the whole session to claim credits for the course.

  • The first hour will include 4 short presentations with time for questions on: “Leverage in-network services to meet the needs of our patients" in the four settings: Post-Acute Network (SNFs), Palliative Care, Hospice and Homecare.
  • The second hour will include two teaching stations in which participants will work through two case scenarios to practice learning on: 1. “Deprescribing medications for an older adult with multiple co-morbid medical conditions” 2.“Transitions of care for older adults whose preference is to return home but don’t have support”

Target Audience

This course is intended for physicians- MDs/DOs, advanced practice providers (NPs, PAs, CNs), PharmD, and RNs

Learning Objectives

At the end of this session, learners should be able to:

  1. Identify Post-Acute services and deprescribing process for older adults to improve outcomes during the transition period by passing a post-test with a minimum score of 80%
