Leading Aurora’s Evolution to Value Based Care: Developing People, Interventions and Analytics

This 30 minute online course will introduce participants to population health management principles and the initiatives under way at Aurora to better care for our patients. 

Target Audience

This course was developed for all clinical staff including physicians, pharmacists, advance practice providers, nurses, and support caregivers.

Learning Objectives

At the end of this session, learners should be able to:

  1. Identify the differences between volume-based care and value-based care
  2. Define ‘risk’ populations for Advocate Aurora Health 
  3. Identify two changes that value-based care will mean for clinical practice
  4. Give an example of a population health intervention based on patient risk stratification
  5. Give an example of coordinated team-based care that helps our patients
